An MK operative who died in police custody in Sasolburg, Orange Free State (OFS), on 8 April 1994. It is alleged that he was shot in a staged suicide to prevent him from revealing information about Security Branch activities. Mr Vilakazi had reportedly been detained and tortured in 1992 in a failed attempt to make him an informer.
